Abstract

ABSTRACT: The Patient-Centered Medical Home (PCMH) is a new care model that reorganizes primary care to improve access, coordination, quality, satisfaction, and comprehensive patient-centered care. Nurse practitioners should understand the PCMH concept, appraise the evidence, and become leaders in this transformation.
