Cuban public health: living the marxist dream.

"In Cuba, citizen well-being is regarded as the result of three interrelated components: the physical environment, which includes the climate, setting, housing, food, and water; the social environment, which includes education, occupation, relationships, and everyday demands; and personal traits, such as age, sex, immunity, genetic background, and behavioral habits. Cuba’s social construction of well-being and disability directly influences the public health methods that have achieved such impressive outcomes.
A few hours of discussion with Cuban health officials left no doubt that some of our most fundamental rights have no place in Cuba’s Marxist–Leninist (Soviet-style) health care system. Cuban governmental control of the health care system is maintained by the country’s leadership and official organs, such as neighborhood polyclinics, including all revolutionary medical services. Alignment within and among government- controlled processes is strong and relatively stable. The public health program is designed to prevent the spread of disease and promote a healthy population and workforce, which are seen as essential features of revolutionary social order. Public health noncompliance is perceived to be a direct threat to national security."
