The issue of unsafe injection medication practices is longstanding. Efforts throughout medicine involving physicians, nurses, and pharmacists have identified unsafe injection practices as a continuing concern for patient safety around the world [1][2][3] and [4]. Despite well recognized risks and concerns regarding unsafe medication practices across settings and health care providers, the incidences and practices continue.
